This employee implements and refines the program, and screens, educates, and orients Medicare beneficiaries for participation. Other duties include identifying Medicare fee-for-service beneficiaries discharging outside of our service area and providing a Care Transitions Intervention enhanced visit in the hospital and follow-up phone calls once they return home. This position also provides a traditional home visit intervention to beneficiaries discharging locally.
